Description
Provides comprehensive primary care services, including preventive care, well adult exams and physicals, diagnostic testing, and management of acute and chronic illnesses. Additional services include dietitian consultations, psychiatric consultations, health education, and medication management, with recommendations for ongoing psychiatric care as needed. A social worker is available to assist patients in accessing community services and resources and to help address barriers that may interfere with access to health care.
