Adults, children and infants with a prognosis of six months or less if the disease runs its normal course.  Eligibility is determined in consultation with the patient's physician.  Must be seeking comfort rather than curative care. Must live in King County, Pierce County or South Snohomish County.
M-F, 8am-4:30pm. A nurse is on call 24 hours daily.

Interpreter services

Call and ask to speak with the Intake Department for intake or referral. Referrals are accepted from physicians, patients, family members and health or social services personnel.  A plan for patient care will follow the initial home visit.  Physician's order is necessary to initiate services.

Medicare, WA Apple Health (Medicaid) and other third party payers are accepted. No one is refused care because of inability to pay.

Description

Provides end-of-life care for children and adults living with life-limiting illnesses and their families. Services are provided in patient's home or residential care setting. Professional health care providers offer medical expertise, symptom management, pain control and promote comfort. Team includes nurses, medical social workers, occupational therapists, home health aides, chaplains, pharmacist, medical director, volunteers and bereavement counselors. Prepared to address veterans' unique end-of-life care needs. Services include bereavement follow-up support for 15 months after a death.

Coordinates grief support groups for children and adults. Also holds a grief support group for those with a developmental disability. Offers a summer camp for children affected by the death of a loved one.
