Offers primary and specialty care for individuals and families such as annual exams and chronic disease management. Same day/next day appointments are available for minor illnesses such as sore throats, ear pain, burns, cuts, sprains, etc.   Affordable Access Program: Offers primary care services for a flat monthly rate of $40 plus a $5 visit fee. This plan does not provide for hospitalization costs, major surgery, dialysis, radiology (X-ray, CT, MRI etc), rehabilitation services, procedures requiring general anesthesia, or any similar advanced procedures, services, or supplies. This plan does not include the costs of laboratory tests performed outside the clinic. Prescriptions are written for lower cost treatment options; cost of medication is not included.   Behavioral Health Services: Offers short-term consultation and counseling to clinic patients. Problems that commonly affect physical health, including depression, anxiety and stress, are the primary focus of the service.

Fee

Accepts Medicare, WA Apple Health (Medicaid) and private health insurance.   Affordable Access Program: Initial membership fee is $125, which covers the one-time application fee of $45, and the first two months of membership. $40 per member per month; $10 visit fee for primary care; $20 visit fee for specialty care.   Monthly fee must be processed as a credit card transaction.

Eligibility

No restrictions.   Affordable Access Program not designed for people with WA Apple Health (Medicaid).
