Homeless Medical Care offered by Neighborcare Health's Ballard Clinic

Provides a health care home for people experiencing homelessness or unstably housed adults as well as adults in supportive housing. Enrolled clients can additionally access behavioral health, social work and many more services to care for the whole person. Medical care includes health check-ups and screenings to help catch issues before they become serious.   Teams can also help take care of specific health conditions, such as high blood pressure, asthma or diabetes. Provides confidential screening, treatment and counseling for sexually transmitted infections (STIs), including HIV. Refers to specialty care services.

Application process

Call for appointment.   Drop-in appointments may be available; please call ahead if possible. Only available up to 90 minutes prior to close.  

Fee

No cost to patients experiencing homelessness. Offers sliding scale fees. Accepts WA Apple Health (Medicaid), Medicare, and most health insurance plans. Insurance not required.

Eligibility

Adults ages 18 and older who are experiencing homelessness or living without safe, stable and secure housing, and youth ages 12-26 experiencing homelessness or unstably housed. Also serves people who live in permanent supportive housing.
