Welcome Home Program offered at Lifelong
Delivers healthy, home-cooked meals to participants' homes within 24 hours of discharge from the hospital. Includes 10 nutritious meals specialized for an individual's unique medical diagnosis. Also includes preparation instructions, a booklet prepared by registered dietitians on how to recover. Meals are cooked in a commercial kitchen, frozen and delivered weekly. Meals meet the guidelines of the American Heart Association and the American Diabetes Association. Also has special meals to provide for dietary restrictions (please see below). Includes wellness check phone calls over 30 days from the first meal delivery. Meal Options: - Healthy Standard - this is our regular meal category, but it is also appropriate for anyone with diabetes or heart disease. - Easy Digestion - this category is designed to meet the needs of those struggling with on-going diarrhea, GERD, or gastroparesis. - Dialysis Care - this category meets the specialized needs of our clients on kidney dialysis. - No Gluten/No Dairy - these meals are the best choice for those with gluten intolerance, celiac disease, or lactose intolerance. - No Pork/No Beef - if you prefer to avoid beef or pork but enjoy chicken and fish, this may be a good category for you. - Vegetarian - these meals contain NO meat, fish, or poultry (but are not vegan). For clients who struggle with chewing or swallowing, any of the above meal categories can be made into a soft texture (very finely chopped).

Call for more information and to verify service area. Referrals accepted from self, doctors, social workers, or family members. Those looking to set up home food delivery for future hospital discharge can prepare the paperwork ahead of time, but delivery will be set up only after client is discharged.

Eligibility
Must be living in South King County and be coming home from a hospital, nursing home or other inpatient facility. Service area boundaries include SeaTac, Des Moines, Normandy Park, Burien, Federal Way, Auburn, Algona, Pacific, Renton, Kent, Covington, Maple Valley, Black Diamond, Ravensdale, Enumclaw, parts of Tukwila, parts of unincorporated King County and parts of South Seattle. No income requirements.
