Unpaid adult, family caregivers of an African-American elder, age 60 or older or an adult 18 or older with a disability. Focuses on caregivers living in Seattle and South King County.   Cannot serve those on the Eastside and NE King County including Bothell and Kenmore.
M-F, 8am-4:30pm.

Caregiver support services may include:   -  Individualized comprehensive needs assessments and develop care plans -  In-home or office visits within King county -  Referrals to support groups, counseling and other resources -  Limited respite for the caregiver

The African American Elders Program serves African American elders in Central and Southeast Seattle and South King County. The program identifies frail, isolated and hard-to-serve African Americans and assists them in accessing and receiving needed social and health services through ongoing case management, support and referrals.
