Offers outreach, comprehensive assessment and early intervention for aging adults who are escalating toward crisis. Serves adults in King County who are ages 55 and older. Does not provide emergency services and is not intended for crisis situations.
Offers outreach, biopsychosocial needs assessment, and tailored referrals to support services for older adults who may be at risk due to behavioral health issues. Does not provide emergency services or crisis response. Please consider eligibility carefully before giving out contact info and note that the program preference is to take referrals from professional providers (e.g., primary care or social services) or those directly involved in assisting an at-risk elder (e.g., caregiver or family member).

Helps older adults and ADS Medicaid long-term care case management clients through culturally sensitive, holistic consultation and care. Provides counseling services, in-home visits, screenings, and treatment plan development and coordination.

- GAIN-SS screenings
- Treatment plan development and coordination
- Individual counseling
- In-home visits for individualized care
- Information and referral for potential clients who are under the age of 60, must be referred by ADS case managers

Primary Care offered by Harborview Medical Center's Senior Care Clinic
Provides primary care for patients 65 years and older who also have a geriatric syndrome such as cognitive impairment, falls and gait instability, frailty or functional decline.
Provides primary care to older adults with complex medical issues. Services include
- Treatment and management of acute and chronic diseases
- Physical exams
- Blood tests
- Nutrition
- Foot care
- Behavioral health
- Care management
Staff also provides information about safety issues relevant to seniors, such as
- Using medications safely
- Conditions such as hypertension and diabetes.
Provides consultative care and referrals to other disciplines as needed for patients concerned about osteoporosis and other skeletal health issues.
