Federal program for navigating the Long Term Care (LTC) services and processes so that the individual may receive any desired help with meeting their personal care needs including getting dressed, driving to appointments, or making meals. Offers Pathfinder services to help find providers in the area as well as different types of services depending on special circumstances, needs, or demographics.

Compares the cost of long term care and long term care insurance by regions.

Provides guidance to individuals to help them make informed choices about supports and services in order to live independently in the community.  Works with the individual to explore options that can help them live independently longer in a manner that is safe and meaningful to them.  An options counselor can meet with the older adult one on one to:   -  Develop a personalized action plan -  Assess and refer to additional resources -  Provide limited assistance in filling out paperwork -  Can provide limited phone-based advocacy to connect client to resources  During visit CANNOT provide: -  Housework -  Errand running -  Personal care services -  Transportation -  Legal representation or advice

Specialists can meet one-on-one.
