Intensive case management services assist immigrants and refugees in overcoming barriers to self-sufficiency that are caused by difficulty in accessing and appropriately utilizing the health and human service systems.

Refugee and Immigrant Health Advocacy Project offered at Center for MultiCultural Health

Case managers provide assistance in accessing basic and emergency resources, naturalization assistance, individual ESL instruction, public benefits application procedures, health insurance programs and health advocacy.

Programs that develop plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual. Case management is a collaborative process characterized by communication, advocacy and resource management to promote high quality, cost-effective interventions and outcomes.

Case/Care Management

County or state offices that accept Food Stamp applications, determine eligibility for the Food Stamp program and allotments, and issue Food Stamp EBT cards which are presented at the grocery checkout counter when purchasing food. Certified households receive their EBT card and instructions for setting up a PIN number within 30 days of the date their application was filed. Expedited food stamps are available within seven days for people who are in an emergency situation and whose income and spendable resources for that month are within specified limits. Also included are other programs that help people prepare and file Food Stamp/SNAP applications and/or are authorized to do eligibility determinations for the program.

Food Stamps/SNAP Applications

Organizations that are dedicated to protecting and improving the quality and affordability of health and long term care for all individuals and advocating for changes in the health care system that will achieve these goals; or which advocate more broadly for legislation and other measures such as bans on smoking that have a positive impact on public health. Specific issues may include extending the reach of childhood immunization programs, expanding services for people with specific disabilities or health conditions, funding medical research, supporting or opposing stem cell research, establishing the liability of managed health care organizations and nursing facilities for the consequences of their decisions, providing a prescription drug benefit for the elderly, passing a powerful patient's bill of rights, advocating for universal health care or taking a position on the merits of specific medical procedures or forms of treatment.

Health Related Advocacy Groups

Programs that provide assistance for non-citizens who are having difficulty understanding, accessing, and/or are unaware of government-sponsored entitlement programs and social services for which they are eligible including housing programs, employment assistance and job training, medical assistance, and other government entitlements. The program may help these individuals understand the eligibility criteria for benefits and services, the benefits/services available, the payment process and the rights of beneficiaries; provide consultation and advice; help them complete application forms; negotiate on their behalf with benefits administration and social service staff; and/or represent them in administrative processes or judicial litigation. Included are benefits counseling organizations that offer a range of advocacy services and legal aid programs that offer more formalized legal assistance.

Immigrant Benefits Assistance

Government-appointed nonprofit organizations, frequently associated with the faith community, that provide resettlement services through a network of local affiliates for refugee populations designated by the U.S. Department of State. These organizations take the lead in helping newly arrived refugees or refugee families make an adjustment to their new home and community. Services provided by local affiliates may include reception for arriving refugees; provision of a "welcome kit" of immediate necessities; periodic visits with the family during the early days of their transition; collection and provision of household goods; cultural orientation and ESL classes; and assistance in locating housing and employment, using public transportation and identifying other needed services.

Refugee Resettlement Services

Programs that offer opportunities for non-English-speaking and limited-English-speaking adults to learn listening, speaking, reading and writing skills with an emphasis on developing the level of communication competence that is essential for adults who are living in a setting in which English is the primary language. Also included are programs that provide English language instruction for younger people.

English as a Second Language

Programs that provide a wide variety of legal and supportive services (which may include information, counseling, application completion and filing assistance and/or legal representation, when required) for immigrants, nonimmigrant visa applicants, asylum seekers and lawful permanent residents who are seeking naturalization. Issues may include procedures for obtaining student, visitor and employment-based visas; family immigration; asylee status; lawful permanent residence status; and/or citizenship.
