This plan does not provide for hospitalization costs, major surgery, dialysis, radiology (X-ray, CT, MRI etc.), rehabilitation services, procedures requiring general anesthesia, or any similar advanced procedures, services, or supplies. This plan does not include the costs of laboratory tests performed outside the clinic. Prescriptions are written for lower cost treatment options; cost of medication is not included.   Behavioral Health Services: Offers short-term consultation and counseling to clinic patients. Problems that commonly affect physical health, including depression, anxiety and stress, are the primary focus of the service.
