Delivers hospice services in homes, nursing homes, adult family homes, assisted living facilities, the Hospice Center and contracted hospitals.  Services include intermittent nursing care, 24-hour on-call support services, social services, hospice aides, trained volunteers, spiritual counseling and bereavement services.   Acute inpatient hospice care is available at the Evergreen Hospice Center and several contracted hospitals.   Respite care is available for caregivers at contracted nursing homes.

Professional health care providers offer medical expertise, symptom management, pain control and promote comfort. Team includes nurses, medical social workers, occupational therapists, home health aides, chaplains, pharmacist, medical director, volunteers and bereavement counselors. Prepared to address veterans' unique end-of-life care needs. Services include bereavement follow-up support for 15 months after a death.

Lists accreditations and qualifications of member agencies. Provides information about selecting the right provider and standard billing and payment practices.
